The three dot combo is simply stacking your 3 lines of dots on a creature. At level 16 it is Heat Blood, Engulfing Darkness, and Heart Flutter. At level 34 it's Boil Blood, Dooming Darkness, and i think it's scourge maybe not it's the poison dot though I can't remember name right now.
